Does this rack come with mounting instructions? How high from the floor should the bottom piece be? How much distance does there need to be between the two rack pieces?

These racks are very simple to mount and you can mount them at any height that you please. I would recommend that you measure your cue sticks to determine the best length for the top and lower rack to be spaced apart.
